Requests for De-Identified Information and Limited Data Sets

Harris County Hospital District’s HIPAA Privacy Policies # 3.11.308, De-Identification of Protected Health Information (PHI) and # 3.11.309, Use and Disclosure of Limited Data Sets both contain suggestions for the use of certain forms by which an individual may request either de-identified information or limited data sets as defined by the Health Insurance Portability and Accountability Act of 1996 (HIPAA). 

The forms used to request Limited Data Sets or De-Identified Data include the “Information Technology Report Request” and/or the “Health Information Management Chart Request Form”.  The request forms can be downloaded, printed, completed and submitted to the appropriate department (determined by the requestor’s purpose) for approval and processing.  The Information Technology Report Request is used to request reports generated by the Information Technology department from electronic data maintained by the Hospital District.  These reports may be in the form of de-identified data or limited data sets (as defined by HIPAA).   

The Health Information Management Chart Request Form is used to request that Medical Records/Charts be retrieved from the Health Information Management Department and made available for inspection by the requestor.  In certain instances, copies of portions of the Medical Records may also be provided.
