Health Care for the Homeless

In an effort to improve healthcare service to the homeless, and enhance access to existing Harris County Hospital District healthcare services, HCHD provides a variety of preventive and primary healthcare services to homeless individuals at 13 shelters and through an additional mobile unit. The program also provides on-site case management and HCHD Gold Card registration services.

Through a number of strategic partnerships, the Health Care for the Homeless program also provides on-site mental health and substance abuse counseling.

This program enables healthcare problems to be detected and treated early, decreases non-emergent use of emergency centers, reduces and/or eliminates many barriers to health care experienced by homeless individuals, increases immunization rates of homeless children, increases tuberculin screening and follow-up and increases health promotion and prevention education.
