Birth Certificates

You may receive a temporary birth fact sheet from the business office at the hospital. However, it is important to remember that a birth fact sheet is not an “official” birth certificate.

To obtain an official birth certificate, you must contact the City of Houston’s Bureau of Vital Statistics, located at 8000 North Stadium Drive in Houston. You should allow 6-8 weeks after the birth of your baby before requesting a copy of the certificate.

To receive your baby's birth fact sheet, you will need the pink copy of your baby's discharge papers or your baby's medical record number. Please allow 7 days after completing the birth certificate information before requesting a birth fact sheet.

There is a $1.00 fee for the baby’s birth fact sheet. The hospital business office is open Monday through Friday, 8:30 a.m. to 4:00 p.m.
